Many managed
care plans will allow a routine eye exam without referral once every one-two
years, depending on the plan. You will need a referral (authorization) from
your Primary Care Provider for additional visits and problems. If your
managed care plan requires a referral (authorization), please allow your
Primary Care Provider time to prepare and forward this required referral so
that your visit will be considered by your insurance. It is your
responsibility to obtain the referral authorization. We will assist you upon
request. When you come to
the office for your visit, please bring your insurance cards with you.
Co-payments are expected at the time of service. Our billing
office will be glad to answer any questions for you.
